Use of Pictures: I understand my youth may appear in pictures that are taken by Maranatha Church and give my permission for their use in promotional materials.

I understand I am required to inform a Pastor at Maranatha Church immediately of any change in the information presented, and this agreement is valid until revoked in writing.
